Phase IV: Advanced Return to Activity & Maintenance (Weeks 12+)
Goals:
- Restore full strength, power, and endurance relative to the patient's baseline or sport-specific demands.
- Transition to an independent home maintenance program for range of motion and conditioning.
- Gradual return to unrestricted work, recreational activities, or competitive sports.
Precautions & Restrictions:
- Pain and fatigue should guide the progression of loading.
- Avoid high-impact or sudden heavy loading until cleared by the surgeon.
Suggested Exercises:
- Sport-specific or work-specific plyometrics and functional training.
- Advanced strength progression (eccentric training, multi-planar movements).
- Ongoing daily home ROM stretching to prevent recurrence of capsular tightness.
Criteria to Progress:
- Elbow strength at least 85-90% compared to the contralateral side.
- Pain-free participation in daily tasks and recreational activities.
- Clearance from the orthopedic surgeon for full activity.
